SUBJECT: NRC INSPECTION REPORT NOS. 50-327/86-48 AND 50-228/86-48

This refers to the Nuclear Regulatory Concission (NRC) inspection conducted by

D. P. Falconer on September 8-12, 1986. The inspection included a review of

activities authorized for your Sequoyah facility. At the conclusion of the

inspection, the findings were discussed with those members of your staff

identified in the enclosed inspection report.

Areas examined during the inspection are identified in the report. Within these

areas, the inspection consisted of selective examinations of procedures and

representative records, interviews with personnel, and observation of activities

in progress.

The inspection revealed no violation of NRC requirements. However, certain

activities having safety significance appeared to deviate from a commitment

to the NRC. The deviation and elements to be included in your response are

described in the enclosed Notice of Deviation.

In addition to the need for corrective action regarding the specific matters

identified in the enclosed Notice, we are concerned about the implementation

of your TVA Nuclear Performance Plan that permitted this situation to develop.

Consequently, your response should describe those particular '

actions taken or

planned to improve the effectiveness of your program.

Your attention is invited to an unresolved item identified in the inspection

report. This matter will be pursued during future inspections.

Your attention is also invited to paragraph 5 of the enclosed report which

addresses general conclusions associated with this inspection and licensee

actions which are considered necessary to be completed prior to unit restart.

In accordance with Section 2.790 of the NRC's " Rules of Practice,"' Part 2,

Title 10, Code of Federal Regulations, a copy of this letter and its enclosure

will be placed in the NRC Public Document Room.
