-Violation:

10 CFR 50, Appendix B, Criterion VI requires that neasures shall be established to control the issuance of drawings affecting quality. The measures shall assure that drawings, including changes, are reviewed for adequacy and approved for release.

The licensee's procedure no. 5000-A0it-7312.02, titled As Built Drawings requires updating of drawings'to incorporate changes to the plant configuration. The licensee's standard DS-010, titled Checking Drawings also contains provisions to reference all affected drawings.

Contrary to the above, control roon drawing, GE 237E726, was not updated to incorporate the plant nodification which installed blind spectacle flanges that provide containnent isolation for the main steam drain. The work package BA 323402 and Safety Evaluation tio. 323402-001 Rev. 1, also neglected to reference this applicable drawing.

Response-Clarification:

Flanges Y-1-57 and Y-1-58 were incorporated in revision 34 of drawing GE-237-E-726 under authority of FCil C04962 dated February 26, 1987. The cause for the initial failure was identified in the finding eg; the nodification package did not identify the drawing as affected. In addition, the flange information is duplicated on two drawings, BR-2002 and GE-237-E-726. The checker of drawing BR-2002, revision 25 (which did incorporate the change) provided correct notification in a timely manner.

However, he failed at turnover to identify the subject drawing as being affected.

Cause:

GPUll believes this is an isolated occurrence. CPUll judges that the principal cause was the failure of the modification package to identify the directly affected reference drawing. In addition, the Design and Draf ting staff failed to recognize that GE-237-E-726 required revision when they completed the As-Built docunentation. Technical Functions Engineering Services has taken steps to ensure that the requirenents of DS-002, section 5.2.1.5.e are strictly enforced.

1. Drafting Standard DS-002 has been revised to require that construction drawings must all carry a note stating which other drawings are affected by construction of the subject drawing. In order to ensure no nisunderstanding, the wording of the note has been prescribed. Where no drawings are affected the note is still required and will state "none".

Additionally, GPUN had previously initiated a drawing update program intended in part to reduce the amount of redundant information contained'

-in controlled drawings. This program is ongoing and will continue into 1988. This program should also serve to reduce the-likelihood of repetition of this violation.

1 Future Prevention:

GPUN judges that the imediate corrective action steps described above will also minimize further violations.
