Dear Mr. Paperiello:

Toledo Edison provides this letter to outline the inspection plan which will be implemented to resolve concerns regarding the potential effects abandoned drilled anchor bolt holes behind basepintes may have on the design margin of installed anchor bolts. Toledo Edison's position regarding this issue was previously submitted on June 18, 1986 (Serial 1-644).

To alleviate concerns regarding potential reductions in design margin for anchor bolts due to adjacent empty holes, an inspection plan will be implemented by the end of the fifth refueling outage. The inspection will be based upon the following:

A sample of anchor bolt installations will be inspected to provide a 95% confidence level that 95% or greater of the installed anchor bolts have factors of safety as required by IE Bulletin 79-02.

The sample will be selected from a cross section of the piping I

systems in the plant.

The inspected bolts will be relatively highly loaded. This criterion remains to be defined and will be specified within the inspection plan.

The ASCE paper titled, "Effect on Abandoned Holes on Capacity of Wedge Bolts," will be utilized as part of the basis for sample acceptance criteria.

This sample inspection shall be completed prior to the end of the fifth refueling outage. Toledo Edison intends to start implementing this program as plant conditions permit, while the unit is on line.
