Consequently} you are required to respond to this violationcin writing, in

accordance with the provisions of Section 2.201 of the NRC?s-" Rules of

be-based on the specifics contained in the enclosed Notice of Violation.

As we noted in the February 20, 1987, enforcement conference, we'are concerned

by the findings of this inspection. Additional management attention is needed

a

to assure.that the data and' calculations used in surveillance ~and' calibration;

procedures are accurate. We are al~so' concerned with the~ management decision to'

'

take :the plant to an indicated power of 100 percent on' January 19 and 20,1987.-

'

'Your decision to not accept as valid the initially calculated RCS total flow

rate on January 14 was based upon a" belief that one or more of the parameters

~

used in'that' calculation was measured by out-of-calibration instrumentation..

The' decision toJincrease power should have considered the possibility that the

,

<

Lsame errant instruments could also result in erroneous power level indication.

(In fact this was thezcase and as a. result you. exceeded your licensed power

level albeit by a1small margin (less than 0.4 percent), for a period of about

,

30. hours'on January 19 and 20.-

In addition to the corrective action required by the enclosed Notice of

Violation,1you are also requested to comment on the assurance you have that

.other critical. instrumentation are not providing errant indications because

of inappropriate set point data or constants used in the calibration of these

'

.. instruments. You should also address the actions you have taken regarding the

.Ldecision'which was made to increase power,on January 19, 1987. Also, your
