50-266 Unit Name Point Beach Unit 1 Date May 7, 1987 Completed By C. W. Krause Telephone 414/221-2001 On April 3, 1987 at 7:00 PM, a normal power reduction commenced as scheduled for the annual refueling and maintenance outage.
The generator was off line at 1:00 AM on April 4.
At 5:19 AM on April 4, while performing end-of-life nuclear physics testing, an electrical short occurred as some test equipment was being connected to the rod control system.
This caused several of the control rods to drop.
The control operator manually tripped the reactor.
As a result of the manual trip, LER 87-001 was issued.
On April 7, at 8:00 PM, containment pressurization commenced for the integrated leak rate test (ILRT).
Following completion of the ILRT, depressurization began and was completed at 5:00 PM on April 9, allowing other outage work to begin.
Ultrasonic testing of the reactor vessel welds, required during the ten-year examination, began during the period.
The upflow modification of the core barrel commenced as scheduled during the period.
Safety-related maintenance performed during the period includes miscellaneous repairs of safety-related instrumentation found during refueling calibrations, and rebuilding of a motor operator on a containment spray isolation valve.
On April 9, 1987, it was determined that the ILRT as-found results did not meet the acceptance criteria.
As a result, LER 87-002 will be issued.
On April 6, 1987, local leak rate testing results for a containment isolation valve did not meet the acceptance criteria.
As a result LER 87-003 will be issued.
