50-301 Unit Name Point Beach Unit 2 Date November 7, 1986 Completed By C. W. Krause Telephone 414/221-2001 Unit 2 continued its annual refueling and maintenance outage during the period.
On October 2, 1986, the containment integrated leak rate test was completed with satisfactory results.
The containment was subsequently depressurized.
j Other work performed during the period includes installation of steam generator nozzle dams, steam generator eddy current testing, inspection of the interior surface of the reactor vessel, and continuing work on the reactor vessel upflow modification.
While performing the Type "C" testing, it was noted that the containment isolation valve on the component cooling water line to the reactor coolant pumps did not meet the leak rate criteria.
LER 86-005 was issued due to this event.
On October 2, 1986, a reactor trip signal was generated due to an instrument bus voltage spike caused by the failure of a shunt resistor in an auxiliary feedwater discharge valve operator.
On October 4, 1986, another reactor trip signal was generated as a result of electrical cable testing associated with installation of a new wide range nuclear instrument detector.
On October 8, an additional reactor trip signal was generated as a result of a voltage fluctuation caused by erratic operation of an instrument bus inverter.
The inverter was subsequently repaired.
All three of these events are being reported in LER 86-006 which will be issued in the near future.
On October 16, it was discovered that two of the four pressurizer pressure transmitters were out of calibration in the non-conservative direction.
This is being reported in LER 86-007.
Safety-related maintenance completed during the period includes overhaul of motor operators on valves in the safety injection i
system and repair of the manway leak on the "A" steam generator which had been furmanited during operation.
