Thank you for your letter dated September 25, 1986 informing us of the steps
you have taken to correct items which we brought to your attention in our
letter dated July 7, 1986.
Your corrective actions will be verified during
our next inspection.

You are reminded that your response to the NRC Notice of Violation should have

been submitted within thirty (30) days of the date of receipt of the Notice.

The official response to our Notice was not received until October 23, 1986, a
period of one hundred and seven (107) days.
We also bring to your attention that License Condition 1 of your license
specifies 5 and S Electric, Inc. and not the current name of your company,
Swanson General Contractor, Inc.
The requirement for you to keep your license
current, and to submit an application to the Region V office for a license
amendment, was explained by Mr. David D. Skov of our office to Mr. Richard

Jensen of your staff by telephone on September 8,1986.
However, a check of
our records indicates that we have not yet received the required application.
This matter should be resolved promptly in order to avoid further
noncompliance with your license.
A check made out to the NRC to cover the
appropriate amendment fee should accompany your application.
