SUMMARY

__OF FINDINGS ON OPERATOR QUALIFICATIONS

f. Approximately 502 of candidates who have taken NRC initial licensing exam i have failed.

l L Essentially no prior BWR operating experience on Grand Gulf staff. Quali fi-cations of GE advisors uncertain. l A Operator requalification training program was not insplemented until April 1983.

Initial NRC exams were given in October 1981.

4. MRC evaluation of W&L requal program in August 1983 revealed problems. One cf three operators failed our portion of the exam. Their exams were judged to s he too easy.

5. Significant weaknesses identified for three of ten operators given walkthru
evaluations by NRC in early November 1983,
i. Certifications on qual cards have been found to be inconplete and have apparently not etways been based on proper evaluations.
7. Operator weaknesses identified by Grand Gulf exams have not always been corrected.

E. Many deficiencies have been found in the ir.anagement cf the Grand Gulf operator training program.

- The MP&L Training Dept. is understaffed. Current staff includes only two MP&L employees. There has been little oversite or appraisal of Quadrexinstructors by W&L.

- License applications have overstated the amount of training given l t to applicants. .

- Training records are incomplete.

1. Qualification Cards

' - Revised guidance on qual card implementation issued

- Qual card reviewed by shift supervisor (SROS) ,

- Developing NRC style Matrix for signoffs of important attributes

- Matrix includes theory, practical factors, system knowledge, technical specification

- Signoffs to be accomplished by shift supervisors (SROS)

- Two classes: One on training; one on shift

2. Relief on Susp_e_nded Licenses

- Issue licenses withheld after completion of qual cards (4 licenses)?

- Reinstate three licenses af ter completion of qual cards / correction of weaknesses?

- After qualification cards are complete, operator Training Evaluation Board will certify training complete and satisfactory performance on oral exam

~ - Present advisors have three weeks of system training and h&ve been examined

- MP&L will review exams and retrain in weak areas

- MP&L will review qualifications, purpose and use of shift advisors and make determination of adequate training

- Will be accomplished as appropriate in plant or on simulator

- MP&L will review qual cards and identify appropriate locations and how, i.e., walk-thru/ accomplish, the item will be complete

- Short term - all documents retained

- Long term - retain main documents

- Discussed types of documents to be used to adequately show that training is completed

l yftS TO BE RESOLVED 1- Unit is presently in Cold Shutdown. At what point should low power operation be resumed? Is power level a consideration for issuance of ifcenses on hold / reinstatement of licenses?

7. Can operator licenses be issued / reinstated after the individual has been recertified by MP&L?

J. Operators will be taken off-shift to participate in training program.

Should the OTEC Board certify each individual in training prior to placing that individual back on shift?
