This is in response to the management meeting held on January 6,1987, with Mr. J. Taylor, members of his staff, and members of my staff regarding our emergency response concerns for the La Crosse Boiling Water Reactor. Attached is a list of several long-standing NRC concerns that were discussed at the meeting.
As a result of that meeting, we understand that your staff will take prompt and effective action to correct and eliminate the emergency response concerns that were discussed, and any additional concerns that have been previously identified in NRC inspection reports although, not specifically addressed at the meeting. We further understand that your staff will inform us of a date in February 1987 when we can visit the La Crosse facility and review progress on the corrective actions taken.

bo LACBWR Emergency Response Concerns The December 1986 annual exercise at LACBWR raised numerous concerns that have been long-standing issues at the LACBWR facility.
The persistence of some of these concerns are summarized on the following pages. This list does not address all areas of concern raised during the December 1986 exercise nor does it summarize all longstanding issues. The attached information is based on the following reports:

5 Communications / Notifications June 1983 - Routine Emergency Plan has no provision for notifying State and local agencies of an Unusual Event.

December 1986 - Exercise Notifications to the NRC took 55 and 65 minutes. They were not made immediately following notifications to the. State and local agencies, but easily could have been.

Communications with the States of Wisconsin and Minnesota were insufficient in scope and content.
State had to repeatedly call the licensee to obtain information. The communicators had no written message forms to ensure consistent and sufficient information would be provided to the States.

Offsite RMT personnel never checked their pocket dosimeters.
June 1984 - Exercise Offsite RMT personnel never checked their pocket dosimeters.
June 1985 - Exercise Offsite RMT personnel never checked their pocket dosimeters.
December 1986 - Exercise The Offsite RMT was unable to locate the plume. (Exercise Weakness)
Poor RMT practices including: failed to zero survey meter which led to erroneous readings throughout exercise; air sampler was placed under vehicle's raised hood such that radiator fan interfered with air flow through sampler.

Onsite Health Physics Practices December 1983 - Exercise Poor nearsite and inplant survey / sampling practices.

May 1984 - Routine The Emergency Plan was modified in May 1983 to require only one annual Health Physics drill, and it could be in conjunction with the annual exercise.
NRC disagreed. NUREG-0654 states two Health Physics drills should be conducted annually, and not in conjunction with the annual exercise.
