Inspection No. 70-1100/86-04 This refers to the Operational Safety Review conducted by Mr. J. Roth and other members of the NRC staff on August 18-22, 1986 of conditions in your facilities on the Windsor, Connecticut site to determine whether there are potential safety hazards that, when combined with facility operations, could adversely impact upon the health and safety of the public or workers at the facilities.

The inspection consisted of selective examination of procedures, processes, and representative records, interviews with personnel, and observations by members of the team.

Two violations of NRC requirements were identified and are described in Appendix A. Please reply to these in accordance with the instructions in that Appendix. That reply is not subject to clearance by the Office of Management and Budget under the Paperwork Reduction Act of 1980, PL 96-511.

In addition to the violations described in Appendix A, the team observed numerous conditions related to the manufacturing facility (Building 17) and the Windsor Site that warrant the attention of management. We would appreciate a response from you describing the actions you take or plan, relative to the observations and recommendations included in the enclosed inspection report. Further, we request that you maintain documentation relative to your action so that the NRC and other responsible federal agencies can follow your progress in resolving these matters.

We are especially concerned about your management controls with regard to safety programs at your facility, in that we noted there were no corporate or division policies or procedures that provided for the establishment and execution of a safety program. For example, there was no program for the control and handling of combustible material, there was no established fire protection surveillance program, and there was no management audit function (oversight) in the areas of industrial safety, fire safety, emergency planning or training. Combustion Engineering, Inc. 2 knowledge at the Windsor Site concerning the hazards related to the handling and storage of zircalloy waste material. These hazards are discussed in detail in the enclosed report.

In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of this letter, the enclosures, and your response to this letter and Appendix A will be placed in the NRC's Public Document Room.
