Gentlemen:

Subject: Millstone 3 Special Inspection 50-423/86-39

This transmits the findings of the December 29, 1986 - January 7, 1987 special NRC

inspection of the inoperability of the Millstone 3 "B" safety injection (SIH) pump

during the neriod from November 26-30, 1986. On December 1, 1986, the NRC was in-

formed aboLt the pump inoperability and your plan to conduct a thorough investiga-

tion of the circumstances. A special inspection was begun when the resident in-

spector became aware of the results of the investigation during discussions at the

Plant Operations Review Committee (PORC) meeting on December 29, 1986, shortly be-

fore your December 30, 1986 licensee event report (LER) on this matter was sub-

mitted. This event involved exceeding a Limiting Condition for Operation and the

recording of false surveillance test data for a test that was not accomplished.

We concluded that the overall safety function remained capable of being fulfilled

by the second train of emergency core cooling equipment. The inoperable equip-

ment was promptly corrected as soon as it was identified on November 30, 1986.

Also, substantive corrective action has been initiated. During the special in-

spection, our inspectors found all personnel contacted to be fully responsive

and open, including the individual who shut the valve which made the safety

injection pump inoperable and who falsely recorded the data. Although the

specific test involved and the recording of data from that test are not required

by the NRC, the importance of assuring safety equipment operability and proper

recording of data are significant issues that need to be addressed. Therefore,

we are not forwarding a notice of violation for this matter at this time and

will contact you separately to schedule an enforcement conference in the NRC 1
