Gentlemen:
This refers to the inspection conducted by Messrs.'J. G. Luehman, H. F. Bundy,
and T. R. Staker of this office during the period July 1-31, 1986,.of
activities authorized by NRC Operating License NPF-38 for Waterford Ste'am
Electric Station, Unit 3, and to the discussion of our findings with
Mr. R. P. Barkhurst and other members of your staff at the conclusion of the
inspection.
Areas examined during the inspection included plant status; Licensee Event
Report (LER) followup; monthly surveillance; monthly maintenance; licensee
followup on previously identified items; ESF system walkdown; offsite review
committee; onsite review committee; routine inspection; low temperature
overpressurization protection (LTOP); potential generic problems; and review of
special reports. Within these areas, the inspection consisted of selective
examination of procedures and representative records, interviews with
personnel, and observations by the inspectors.
These findings are documented
in the enclosed inspection report.
During this inspection, it was found that certain of your activities were in
violation of NRC requirements.
Consequently, you are required to respond to
these violations, in writing, in accordance with-the provisions of
Section 2.201 of the NRC's " Rules of Practice," Part 2, Title 10, Code of
Federal Regulations.
Your response should be based on the specifics contained
in the Notice of Violation enclosed with this letter.
NRC Violation 382/8615-01 described in the enclosed Notice is similar to a
violation contained in the Notice sent to you by our letter dated January 29,
1986.
Because "similar violations," as described in the NRC Enforcement
Policy, are of significant concern to the NRC, please give particular attention
!
in your response to the identification of the root cause of this problem and
your corrective action to prevent recurrence.
Unresolved items are identified in paragraphs 12 and 13.
