Oyster Creek Nuclear Generating Station Docket No. 50-219 IE Inspection Report 50-219/85-35 Your letter of July 21, 1986 evaluated GPU Nuclear's exception to Notices of Violation A and C(3) cantained in IE Inspection Report 85-35 and concluded that the violations were valid.

GPU Nuclear was requested to provide a supplemental response to our April 18, 1986 submittal on these violations within 30 days of your letter.

This letter fulfills that requirement.

Violation A Your letter documented discussions held between GPU Nuclear and NRC

inspectors subsequent to our original submittal.

You further delineated corrective actions which had been taken by GPU Nuclear and requested a date for completion of all corrective actions.

Full compliance will be achieved when GPU Nuclear Standard 6150-STD-7220.01 is revised, presently projected for November 3,1986 l

10CFR50, Appendix B, Criterion X and Section 6 of the Oyster Creek Operational Quality Assurance Plan require, in part, that inspections be performed to verify conformance with documented instructions, procedures, l

and drawings.

Contrary to the above, as of November 6,1985, QC inspections failed to identify the following deficiencies:

l (2) The adequate partial penetration welds discussed in paragraphs B and C.(2)(a) of this Notice of Violation.

Lanediate Action Instructions have been issued to QC inspectors who are certified to inspect structural welding that:

(1) welds will not be inspected unless a proper weld symbol in accordance with AWS A2.4 is provided on a sketch or drawing; and (2) partial penetration butt welds must have the amount of penetration specified (on the weld symbol) to assure a sufficient effective throat area.

Actions to assure adequate penetration will be a sampling of fit up inspections and a final visual inspection for proper penetration where possible.

Actions to Prevent Recurrence The QC inspectors who are certified to inspect structural welding will be trained on the requirements for inspection of partial penetrations welds and the proper use of weld symbols.

Additionally, the indoctrination for new and contracted inspectors will be revised to incorporate the above actions to assure continued awareness of the corrective action.

Date Full Compliance Will be Achieved Full compliance will be achieved upon completion of training and revision of the indoctrination document presently projected for September 15, 1986.

PURPOSE The purpose of this procedure is to provide the instructions necessary for the notification and activation of the Corporate Emergency Organization.

2.

APPLICABILITY This instruction applies to the Nuclear Operations Department managers and executives who are part of the Corporate Emergency Organization.

3.

RESPONSIBILITIES The Senior Vice President Nuclear Operations, or his/her desig-

nee, is responsible for activation of the Corporate Emergency Plan.

The Corporate Emergency Organization managers are responsible for notification of their support staff as well as other offsite support groups as necessary to respond to the emergency. illustrates the Nuclear Emergency Notification Tree.

4.

INSTRUCTIONS A.

Notification of the Corporate Emergency Organization Atlanta will be 1.

The Nuclear Operations Duty Officar notified by the VEGP site of any emergency situation at VEGP.

He/she will promptly notify the Senior Vice Presi-dent Nuclear Operations, or his/her designee.

2.

In the event of an ALERT or higher emergency classifi-

cation, the Nuclear Operations Duty Officer - Atlanta shall notify, in addition to the Director of Corporate Response, the following:
