NOTICE OF VIOLATION Consumers Power Company Docket No. 50-255 As a result of the inspection conducted during the period July 24 through
' December 31, 1986, and in accordance with the " General Policy and Procedures for NRC Enforcement Actions," 10 CFR Part 2, Appendix C, (1985), the following violation was identified:
Palisades Technical Specification 3.6.1.a requires that " containment integrity shall not be violated unless the reactor is in cold shutdown condition."
Technical Specification 1.4 defines Containment Integrity to include at least one door in each air lock being properly closed and sealed, and all nonautomatic containment isolation valves be closed.
Contrary to the above, with the Unit operating above cold shutdown and with the opening of both containment escape airlock door equalizing valves, due to an equipment design deficiency and operation of the airlock, repeated
-violations of containment integrity occurred over the period of April 1971 through May 1986.
This is a Severity Level IV violation (Supplement 1).
The inspection showed that action had been taken to correct the identified violation and to prevent recurrence. Consequently, no reply to the violation is required and we have no further questions regarding this matter.
