Subject: REQUALIFICATION PROGRAM EVALUATI0ri REPORT NO. 50-029/86-20(0L)

On November 5-7, 1986, the NRC audited the administration of your annual

requalification written and oral examinations ar.d reviewed selected requali-

fication training records. At the exit interview held on November 14, 1986

the preliminary results of these evaluations were discussed.

Areas examined during this evaluation are described in the NRC Region I

Requalification Program Evaluation Report which is enclosed with this letter.

Within the scope of this evaluation, the requalification training program was

found to be marginal.

Several weaknesses were identified in the Training Department's implementation

of the requalification program, including a lack of aggressive follow-up action

to areas of identified operator weakness and an inadequate Training Department

review of contractor prepared examinations. These areas of weakness are docu-

mented in Appendix A. You are requested to respond to these concerns by

explaining what actions have been taken or are planned to improve these areas

of weakness.

In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of

this letter and the enclosure will be placed in the NRC Public Document Room.

APPENDIX A

The effectiveness of the training department in implementing the requirements

of the requalification training program has been identified as marginal.

Specific areas of concerns and the applicable regulatory requirements are

presented below. References in the parenthesis are paragraphs in the Requali-

fication Program Evaluation Report 50-29/86-20 (OL).

1. Training on recent facility and procedural changes is not provided and

evaluated on a timely basis. 10 CFR 55 Appendix A, 3.c, requires that

"each licensed operator and senior operator is cognizant of facility

design changes, procedural changes, and facility license changes."

2. Areas of identified individual or generic weaknesses are not strength-

ened in a timely manner. The requirement to document "... any additional

training administered in areas in which an operator or senior operator

has exhibited deficiencies," is contained in 10 CFR 55 Appendix A, 5.a.

(Section 5)

. 3. The facility review of the annual requalification written examinations is

inadequate to ensure broad coverage and technical accuracy. The written

evaluation is required by 10 CFR 55 Appendix A, 4.a, to " determine areas

in which retraining is needed ..." (Sections 3 and 7)
