DUKE POWER COMPANY McGUIRE NUCLEAR STATION RESPONSE TO VIOLATION IN INSPECTION REPORT 50-369/86-15 AND 50-370/86-15 Violation 50-369/86-15-01, Severity Level IV Technical Specifications 6.8.1 requires that written procedures be established, implemented and maintained covering applicable procedures recommended in Appendix A of Regulatory Guide 1.33, Revision 2, February 1978.
Appendix A. Regulatory Guide 1.33. Paragraph 7.e.1, recommends that the licensee have radiation protection procedures to control access to radiation areas including a radiation work permit system.
Health Physics Manual Section 2.4, Radiation Work Permits, Paragraph 2.4.1, requires the issuance of radiation work permits (RWP) for a particular job in a particular area and on a specific piece of equipment or on a specific component and to delineate the specific protective clothing requirements.
Contrary to the above, specific protective clothing requirements were not delineated on RWP NO. 86-1183 in that verbal upgrades in radiological control requirements had been made without written RWP revisions such that on May 29, 1986, an individual in Unit 1 lower containment, who was following the written RWP, dressed. inappropriately for the radiation hazard that had been determined to be present.
Response
1.
Admission or denial of the alleged violation:
Duke Power agrees that the violation occurred as stated.
2.
Reasons for violation:
The RWP's were not updated because the changes were viewqd as upgrades to requirements, not necessarily of a permanent nature, and could be effectively communicated verbally.
3.
Corrective steps which have been taken and the results achieved:
-The affected RWP's were changed. Health Physics Manual Sectio'n 16.1 (Preparation and Control of RWP's and SRWP's) was amended to require reissuing an RWP when radiological conditions require a change in Health Physics requirements and the change exists for more than a shift.. If a change exists for a shif t or less the requirements may be verbally communicated to involved personnel and noted in the Health Physics Shift Log Book in lieu of reissuing the RWP. This action shall be performed whether

the Health Physics requirements are being made more stringent or being relaxed. Health Physics personnel were informed of the changes.

Corrective steps which will be taken to avoid further violations:
No further action is necessary.

Date when full compliance will be achieved:
McGuire Nuclear Station is presently in full compliance.
