Transmittal of Problems with Selected Elcment Reports in

-Engineering Category of En!ployee Concerns By letter dated January 20, 1987, the staff transmitted a list of problems it identified with element reports in the Operations Category.

In that letter, the staff also stated that it was continuing its review of other element reports, and that it would identify additional concerns as they arose. Therefore, the purpose of this letter is to transmit staff concerns identified for five element reports in the Engineering Category.

It shculd be noted that these five reports are a sample of approximately ten reports received prior to January 1987. They are not a subset of the 80 or more reports received in January 1987.-

It is requested that these element reports he reviewed and, if necessary, appropriately modified.

In its continuing review the staff will provide further comments regarding similar problems with other element reports.

In the meantine the Tennessee Valley Authority should consider these types of problems in your continuing review of element reports..

The concern is that a snubber was installed instead of a rigid support shown on the drawing.

The cercern was validated and the report conclusions noted that TVA could substantiate calculations to substantiate the snubber or replace it with a rigid support.

No where dces the report discuss the cause or identify where TVA has performed audits to ensure that this is the only problem of this type.

Also the request references TVA drawings, telephone conversations, and handbooks that are not part of the report oer se. Therefore, the report is not a

" Stand-alone" docurrcrt.

Feport EN P32.2 (Initial)

This report deals with the physical durability of the soft type insulaticr material widely used at the Watts Bar ard Sequoyah Plants. Although the ccn-cern deals with the service life espect of the insulation, TVA fails to adcress a basic safety issue associated with the insulation. This includes clogging of sumps with pieces cf insulation.

In addition, TVA does not provide reference to the applicable regulations or other staff cuidance.

Report EN 232.2 (Final)

This issue deals with the use of carbon vs stainless steel in drain lines. TV/

justifics rct addressing this issue prior to restart by noting that the use of carbon steel requires a better maintenance program.

It cannct be determined from the report if carbon steel was used where stainless steel was required or if the employee, in his judgement, believed that stairless steel was more apprc-priate than the carbon required in the design.

Report EN 241.4 (Final)

The subject of this report addresses the irprocer installation of /cpheral connectors on the actuators of the eftercency diesel generators. TVA concludes that this issue need not be addressed prior to restart since the conrectors have been replaced. TVA does not address the cause of this problem nor does it discuss if a review hts been conducted to determine if other connector assemblies have been improperly installed, including Amphenal.

Report Eh 224.5 (Final)

This concern deals with the ability of unsupported cables dropping vertically to withstand a seismic event.

In the report, TVA discusses the results of tests which were preforr.ed on similar cables.

However, there is insufficient information to deternine if the tests accurately nicael in-plant conditions.

Even after a conference call with TVA and Cechtel, the staff was unable to determine if the tests were adcquate.

liithout the test report and scheratics, the staff is unable to complete its review.

TFerefore, this report will be the subject of a staff audit.
