Chronology:

08/01/84: Office of Quality Assurance issues audit deviation report citing a lack of design control with regard to calculations 11/85: All employee concerns received by TVA for BFN, SQh, WBN, and BLN except I-85-132-SQN-1 01 /8 6: TVA hires Sargent and Lundy to review electrical calculations 02/04/86: DNE issues Revision 0 of DG loading calculation 03/06/86: NSRS issues investigative report on diesel generator loading.

04/07/86: NSRS issues another report further documenting this lack of design control and determines it to be generic to DNE 04/08/86: Sargent and Lundy releases results of its study which finds that most calculations were either prepared informally or not maintained 09/22/86: Concern I-85-132-SQN-1 received by TVA for SQN 10/16/86: DNE issues ECN L6715 to change DG loading Discussion:

The concerned individual raises the general issues that preparation, management, and control of calculations for electric loads are inadequate. It is asserted that this inadequacy, as it specifically applies to diesel generator loads and margins, has led to the addition of a fifth diesel generator to increase system capacity. The concerns question the ability of the Auxiliary Power System ( APS) to perform its intended safety function.

Lack of documentation of the diesel generator loads and margins has been substantiated by several reports. The first systematic analysis of this issue was NSRS Report I-85-132-SQh ( App. A, 5.c; 03/06/86), which referenced Office of Quality Assurance (00A) Audit 104ad (01/02/87)

. deviation report found there is insufficient design control to ensure that calculations and studies performed by the APS section of the Electrical Engineering Branch (EED) are updated and revised to support design changes. NSRS Report No. I-85-g92-SQN (App. A, 5.a; 04/07/86) further documented this lack of design control and concluded that it was generic to the Division of Nuclear Engineering (DNE). Finally, a Sargent & Lundy (S&L) Report ( App.

A, 5.n; 04/08/86) on the SQN electrical calculations program found that the majority of the calculations were prepared informally by TVA during the design period and were not maintained in a manner that would allow easy retrieval over the life of the plant (the evaluation team has made similar observations during this review).

Sal concluded, however, that "Sequoyah is, in general, typical of industry practice in the time frame involved."

As a result of all the identified deficiencies of SQN calculations, a program was established to generate a minimum set of _ calculations required to support the design bases. To ensure proper maintenance of these calculations, Nuclear Engineering Procedures (NEP) 3.1

" Calculation" and 6.1 " Change Control" have been established. This program is discussed in element reports for Subcategory 205.

Calculation SQN-E3-002, " Diesel Generator Load Analysis," is part of this minimum set of calculations.

The evaluation team reviewed calculation SQN-E3-002 R2 ( App. A, E.r; 08/27/86) to determine the adequacy of the DG capacity. The calculation contains loading sequence tables corresponding to several Design Basis Events (DBE). However, these tables are based on plant configurations (i.e., physically connected and sequenced loads) that are different from the actual "as-built" condition that existed at the time Revision 2 of the calculation was performed.

These analyzed plant configurations were termed " unverified assumptions," and the plant modifications required to reconcile the "as-built" condition with the analyzed configuration are to be implemented per ECN L6715 prior to restart.

Review of the calculations indicated that none of the DBE loading sequences for any of the diesels completely envelop the others and consequently, none of the sequences represents at all times the worst case diesel loading conditions. Therefore, all the sequences were evaluated. Loading simulations were performed for the identified loading sequences to demonstrate adequate loading 1

capability and compliance with Regulatory Guide 1.S. Further, the steady state DG load was calculated and found to be acceptable if credit was taken for operator action of tripping certain long-tem loads. TVA advised on 09/19/86 via teleconference (App. A, 7.b) 1044d (01/02/87)

SPECIAL PROGRAM REVISION NUMBER: 1 PAGE 7 0F 14 that operating procedures exist fcr monitoring steady state DG loads, and a list of equipment that may be tripped to maintain the long-term load within the DG limit is established. hcwever, no evidence exists that these procedures have been revised to include -

the additional loads that may be tripped as a result of revision of the calculation, and no commitment to do so was identified.

Finally, this calculation assumes that Unit'l is in cold shutdown; therefore, this calculation must be revised prior to Unit 1 restart to reflect simultaneous operation of Units 1 and 2. This may lead to further plant modifications.

TVA indicated during the 09/19/86 teleconference ( App. A, 7.b) that the only other DG calculation known to exist prior to SQN-ES-002 is a steady state analysis performed in November 1985. This calculation also identified a need to change plant configuration and formed a basis for (and was superseded by) the current calculation SQN-E3-002. No calculations made prior to November 1985, to demonstrate adequate capacity and margins, were identified.

The fifth DG is being added in accordance with Design Criteria 50N-DC-V-11.1.1 R0 ( App. A, 5.f; 05/16/80). This criterion explicitly states that the additional diesel generator unit is being added to increase plant availability. This is further supported by engineering design drawings which show provisions to ensure that the fif th DG cannot be connected to the APS without disconnecting one of the other four. Therefore, credit for the fifth diesel capacity is not taken.

A.s indicated by the concerned ir.dividual, there is currently no reference to the fifth diesel generator in the licensing documents (e.g., FSAR). TVA personnel advised the evaluator that there is no need to license the fif th diesel generator before restart of Unit 2 (App. A, 7.a and 7.c). Evaluator's concurrence with this conclusion is based on the facts that 1) the fifth diesel is not required for restart of Unit 2, and 2) 10CFR50.71 ( App. A, 6.h) does not require revision of the FSAR until design modifications are inplemented and are to become operational. Licensing issues are addressed in more detail in Sequoyah Element Report 201.5.

Findings:

a. Diesel generator capacity margin is adequate for restart of Unit 2 with four diesel generators available and Unit 1 in cold shutdown. However, modifications must be made to the load configurations per ECN L6715, and operating procedures require updating to reflect the additional long-term loads that may be tripped.

b. Diesel generator loading and margin records had not been adequately maintained. To rectify this, TVA has implemented a program to identify, update, and maintain the minimum calculation set. These calculations will be controlled per HEP 3.1.
c. The fifth DG is not presently included in the SQN licensing documents and is not a licensing issue until it is to be put into operation.
d. The fifth diesel generator is being added to increase plant availability when one of the four DCs is taken out of service, not for additional capacity.

Conclusions:

The employee concerns raised valid issues regarding adequacy of the DG load calculation and, in general, the DG system capacity. The problems associated with these issues have been recognized by TVA and corrective actions initiated. Completion of the Unit 2 changes per ECN L6715, implementation of TVA's long-term calculation program, and future adherence to the new procedures (Nuclear Engineering Procedures and SQN Engineering Procedures) will correct the problems and prevent future recurrence.

10. CORRECTIVE ACTION:

In response to the findings above which require corrective actions as identified by corrective action tracking documents (CATDs) 243.00 SON 01, 243.00 SQA 2 and 243.00 SQh 03 (to be identified as CATD-1, CATD-2 and CATD-3 respectively), TVA has generated corrective action plans (CAPS) as identified by transmittals TCAB-012 dated 11/24/86 and TCAB-043 dated 12/24/86 (App. A, 5.cc). These transmittals have been reviewed and found to contain two Corrective Action Plan of Employee Concern Investigation

, Tracking Checklists, one for each of two CAPS.

The first CAP responds to CATD-1 and CATD-3 and must be completed rior to Unit 2 restart. reviewed to ensure all changes necessary to validate the unverified assumptions have been made. These actions should ensure that

. adequate DG capecity for operation of Unit 2 exists.

The problem described in CATD-3 was " calculation SQN-E3-002 identifies several long-term loads that may be tripped to keep the

.long-term loading within the diesel generator's steady state capabili ty. To date, no evidence of incorporation of these operator actions into operating procedures has been identified."

The TVA solution to this problem was to commit to the revision of Abnormal Operating Instruction 35 ( App. A, 5.ee) " Loss of Offsite Power" to reflect the additional long-term loads that maybe tripped. This action will rectify the described problem.

Finally, the first CAP contains two other appropriate actions: an action to revise SCR SQNEEB8629 to reference CATD-1 and CATD-3 thereby closing the loop in the dccumentation, and an action of continued adherence to NEP 3.1 and SQEP 9 " Review Plan for Electrical Changes" ( App. A, 5.dd) to ensure that future changes are evaluated for DG loading and that the calculation is revised when necessary, thereby preventing recurrence of the concerns. The former action is to be completed prior to restart of Unit 2 and the latter is to be a continuous effort.

The second CAP responds to CATD-2, and its completion is not.
