SUBJECT:

LOSS OF SPECIAL NUCLEAR MATERIAL (FISSION DETECTORS) - UPDATE .

This PN updates PNO-!!s 6-75, issued on October 15, 1986.

A Region !! inspection at Browns Ferry last week confirmed that about 10 grams of uranium-225 contained in five fission chambers apparently was shipped with a radioactive waste package to a Richland Washington, low-level disposal site on March 4,1985. TVA j reported the material missing last week. .

l The inspection also revealed that TVA had inadequate control and accountability proceduref, l for this material, that annual physical inventories were improperly performed, and that numerous transportation regulations were violated as a result of the shipment,of the fission chambers.

The loss of this material is not expected to cause any unnecessary personnel exposure (,r pose an environmental threat. TVA. discussions with Washington State authorities indicated no reasons now to attempt to retrieve the buried package.

TVA has formed an investigation team to review the circumstances of this event. Reglen !!

has enforcement action under review.

TVA issued a press release last week on the loss of the material, and p!sns anothsr announcement today about the formation of the investigation team.

The State of Alabama has been informed.
