REQUEST FOR INFORMATION OIA Report 86-10, provided by Board Notification No. 86-24 refers on page 5 to an issue " Failure to Properly Identify a Spool Piece." This issue is mentioned again on page 6 and page 9.
Page 9 states "...the utility located a mark on the spool piece and showed it to the inspectors." If possible, please identify the size of the piping spool piece, the location of the spool piece within the chemical volume control system (CVCS), the type of material (e.g., stainless steel), and the consequences of a failure of spool piece.
