NRC INSPECTION REPORT #86-48 DOCKET NO.-50-354 HOPE CREEK GENERATING' STATION Public Service Electric and Gas Company (PSE&G) is in receipt of your letter dated November 19, 1986, which transmitted a Notice of Violation concerning a failure to comply with the requirements of 10 CFR 50 Appendix B, Criterion V.
Pursuant to the provisions of 10 CPR 2.201, our response to the Notice of Violation is provided in Attachment 1.

[10 ' CFR 2.201 INFORMATION PUBLIC. SERVICE ELECTRIC AND GAS COMPANY:
HOPE CREEK GENERATING STATION RESPONSE TO NOTICE OF' VIOLATION
' INSPECTION. REPORT NUMBER 50-354/86-48 Your letter dated November 19, 1986 identified a violation which cited a f ailure to comply.with the requirements of 10 CFR 50
- Appendix B, Criterion V-which states,lln part, that " Activities
' af fecting quality shall be prescribed by documented instructions and shall be-accomplished in accordance with 'these instructions."
aAs described in the Notice of. Violation, the covers for several Rosemount transmitters were found to be not torqued to the values specified in our procedure IC-GP.ZZ-053(O), Rev. 3, Maintenance Of-Environmentally Qualified Rosemount Transmitters.
This discrepancy caused these transmitters to be potentially exposed to _ environmental conditions different f rom those for which they were. qualified.

PUBLIC SERVICE ELECTRIC AND GAS COMPANY DOES NOT DISPUTE THE VIOLATION.
2.
THE ROOT CAUSE OF THE VIOLATION WAS INADEQUATE CONTROL OF ACCESS TO THE SUBJECT ROSEMOUNT TRANSMITTERS DURING THE OVERLAP OF THE STARTUP TESTING PROGRAM AND NORMAL STATION
' OPERATING ACTIVITIES.
SOME TRANSMITTERS WERE OPENED DURING STARTUP TEST TROUBLESHOOTING AND THEN RECLOSED WITHOUT PROPER CLEANING OF THE BODY / COVER THREADS AND/OR WITHOUT TORQUE
.VALUE VERIFICATION PER THE APPROVED STATION PROCEDURE.

IMMEDIATE CORRECTIVE ACTIONS:
All' Rosemount transmitters requiring environmental sealing were inspected.
Cover and body threads were cleaned as
-necessary and all covers were properly retorqued..

CORRECTIVE ACTIONS TO PREVENT RECURRENCE:
A3 noted in the. inspection report, the~ procedure called for cleaning of the cover threads prior to installing the cove rs.
Procedure IC-GP.ZZ-053(0) was modified, reenforcing
-the' thread cleaning requirements, to provide additional assurance that proper sealing is achieved at the specified torque. values.
Additionally, all shop instrument technicians have been instructed. in the importance of strict compliance with environmental qualification requirements for Rosemount transmitters.
