(1) Each sealed source containing licensed material, other than Hydrogen 3, j

l with a half-life greater than thirty days and in any form other than gas l

I shall be tested for leakage and/or contamination at intervals not to exceed l

I six months.

In the absence of a certificate from a transferor indicating u

l that a test has been made within six months prior to the transfer, a sealed l

g source received from another person shall not be put into use until tested.

I (2) Notwithstanding the periodic leak test required by this condition, any l

I licensed sealed source is exempt from such leak tests when the source I

1 contains 100 microcuries or less of beta and/or gamma emitting material or I

l 10 microcuries or less of alpha emitting material.

1 (3) The periodic leak test required by this condition does not apply to sealed I

l sources that are stored and not being used. The sources excepted from this I

test shall be tested for leakage prior to any use or transfer to another I

person unless they have been leak tested within six months prior to the 1

date of use or transfer.

B.

The test shall be capable of detecting the presence of 0.005 microcurie of I

radioactive material on the test sample. The test sample shall be taken from I

the sealed source or from the surfaces of the, device in which the sealed source I

is permanently mounted or stored on which"one might expect contamination to I

i accumulate. Records of leak test results shall be kept in units of microcuries l

and maintained for inspection by the Commission.

If the test reveals the presence of 0.005 microcurie o'r more of removable I

contamination, the licensee shall immediately withdraw the sealed source from 1

J use and shall cause it to be decontaminated and repaired or to be disposed of in I

accordance with Commission regulations. A report shall be filed within 5 days l

of the test with the U. S. Nuclear Regulatory Commission, Region I, 631 Park i

Avenue, King of Prussia, Pennsylvania 19406, describing the equipment involved, I

the test results, and the corrective action taken.

The licensee is authorized to collect leak test samples in accordance with the 1

procedures described in the licensee's letter for analysis by Siemens l

Gammasonics. Alternatively, leak test samples may be collected and/or analyzed by other persons specifically authorized by the Commission or an Agreement State l

l to perform such services.

14. Sealed sources containing licensed material shall not be opened.

Except for plutonium contained in a medical device designed for individual human l

application, no plutonium, regardless of form, shall be delivered to a carrier for p

shipment by air transport or transported in an aircraft by the licensee except in 1

j packages the design of which the NRC has specifically approved for transport of I

plutonium by air.

16. Except as specifically provided other.iise by this license, the licensee shall possess I

and use licensed material described in Items 6, 7, and 8 of this license in accordance i

with statements, representations, and procedures contained in application dated I

January 9, 1985. The Nuclear Regulatory Commission's regulations shall govern the i

1 licensee's statements in applications or letters, unless the statements are more I

restrictive than the regulations.
