Gentlemen:

This refers to the routine safety inspection conducted by NRC inspectors,

M. M. Holzmer, L. E. Kanter, P. R. Wohld and P. L. Eng of this of fice on

September 26 through November 17, 1986, of activities at the Zion Nuclear

Generating Station, authorized by Licenses No. DPR-39 and No. DPR-48, and

to the discussion of our findings with Mr. G. Plim1 at the conclusion of

the inspection.

The enclosed copy of our inspection report identifies areas examined during

the inspection. Within these areas, the inspection consisted of a selective

examination of procedures and representative records, observations, and

interviews with personnel.

During this inspection, certain of your activities appeared to be in violation

of NRC requirements, as specified in the enclosed Notice. A written response

is required.

With regards to the July 23, 1983, loss of recirculation flow to the Unit 1

injection tank (BIT), we are concerned about operator awareness of the status

of current out of service (00S) jobs prior to issuing new 00S jobs. More

generally, we are also concerned about operator awareness of the plant status, i

We feel that operators should have available information regarding normal and

abnormal positions of valves, breakers, switches, and other operating control

components, and that this information should be considered prior to making

other necessary changes in the state of these components in order to prevent

causing a safety syste:r. or train from becoming inoperable. Please include in

your response to this letter your plans to review and upgrade, if necessary,

operator awareness of plant status. In addition, the ability of operators to

detect the loss of BIT recirculation flow was adversely affected by burned out

annunciator lamps. Shift lamp checks had been reduced to a daily frequency

because the lamp checks have often resulted in blown fuses. Please include in

your response to this letter the actions you plan to take to resolve the

overcurrent problem associated with annunciator lamp checks.
