CHEM-NUCLEAR CNSI 1-13G CASK DEFECT REPORT DESCRIPTION On January 25, 1987, the 1-13G Cask arrived unloaded at Big Rock Point and was placed into the Spent Fuel Pool. After loading the cask with liner #61, the cask was removed from the fuel pool, drained and decontaminated. On January 26, 1987, while wiping the cask following decontamination, water was noticed seeping from the bottom plate weld. Closer inspection revealed a 4" and a 9" crack in the weld. Gamma Isotropic analysis proved the water to be conteminated and not condensation. At that time, Chem-Nuclear was notified and shipment of the cask was delayed until further investigation.

On January 27, 1987, the weld was inspected using non-destructive testing techniques and results indicated longer cracks than what were observed earlier.

CORRECTIVE ACTIONS On January 27, 1987, Chem-Nuclear was asked to fabricate a " Bottom Support System" to allow lowering the cask back into the fuel pool to permit off-loading liner #61. Our concern was the possibility that the bottom of the cask could fail allowing approximately nine tons of material to impact structures beneath the cask.

On January 31, 1987, the " Bottom Support System" arrived from Chem-Nuclear (Accachment 1).

The cask was prepared to receive the bottom support and on February 1, it was rigged to the cask. With the support accached, the cask was lowered into the fuel pool and liner #61 was removed. The cask was then removed, drained and decontaminated.

On February 2, 1987, the cask was returned to Chem-Nuclear in Barnwell, South Carolina.

During the evolution, a Chem-Nuclear representative Burl G. Fauchier, was on-site for consulting purposes.

Mr. Fauchier is the Cask Maintenance Manager at Barnwell.

CAUSE AND CORRECTIVE ACTIONS Cause of the weld failures was unknown at the time the cask was returned to f

hem-Nuclear.

Responsibility for cause determination and corrective actions for repair should be accomplished by Chem-Nuclear. Consumers Power does not intend on providing any further updates. Chem-Nuclear's corporate addresses is:
