MONTHLY OPERATING ACTIVITIES - NOVEMBER, 1986 s

HIGHLIGHTS:

1 The reporting period began with the\\ reactor.at 90% rated thermal power.

On 11-11-86, theunitinadvertentlyenteredTebhnical Specification 3.0.3 due to both engineered safety features fans being inoperablet One fan was returned to service immediately upon discovery, which removed the unit from Tech. Spec 3.0.3.

On 11-22-86, the unit, automatically' tripped due to operation of the main turbine thrust bearing failure trip.. The trip occurred concurrently with routine testing of'the main turbine control i

valves.

The trip occurred due to a; shift in the' thrust bearing wear detector position.

The thrust bearing. wear detector device position was adjusted and the unit returned to service on

On 11-26-86, an unusual event was dsclared whsn both diesel i

generators became inoperable'at the same time'due to equipment problems.

The unusual event ended after'two hours when one of.

the diesels was declared operable.

s Th'e' reporting, period ended with the reactor at 90% rated thermal

-power.

Gross electrical generation for the month of November was 646,100 MWH.

SUMMARY

11-11-86 At 0623, the manual inlet damper to the #1 ESF fan, HV-AES-1, was found closed.

The damper was immediately re-opened, restoring HV-AES~-lfto operable.

The #2 ESF fan, HV-AES-2, had been removed from service for maintenance at 0605; which caused 1the unit to be in Technical Specification 3.0.3 stat'ement from 0605 to 0623.

The trip occurred concurrently with routine testing of the main turbine control valves.

The trip occurred due to a shift in the thrust bearing wear detector position.

The thrust bearing wear detector device position was adjusted and the unit returned to service on 11-23-86.

All essential equipment operated per design.

At 2240, the reactor was taken critical.

11-23-86 At 0312, the reactor entered Mode 1.

At 0513, the gene *ator was paralleled to the grid.

At 1100, the reactor power increase was stopped at 48%

for a chemistry hold.

At 1707, the reactor power increase to 80% began at 6%/hr.

11-24-86 At 0038, a reactor power increase to 90% began, reaching 90% at 0310.

11-26-86 At 0331, the AB Diesel Generator was declared inoperable when it failed to achieve required output voltage during surveillance testing.

At 1701, the CD Diesel Generator was declared inoperable when it restarted by itself after being shutdown.

At 1707, an unusual event was declared because both l

diesel generators were inoperable.

At 1900, the unusual events was terminated when the AB diesel generator was declared operable.

A relay was found loose on a circuit board in the AB Diesel l

Generator local control panel.

11-27-86 At 0435, the CD Diesel Generator was declared operable.

The solenoid on one of the starting air valves was l

found to have failed, causing that starting air valve to open, which resulted in the diesel restarting.
