Gentlemen:
This refers to the special safety inspection conducted by Messrs. Jeff Holmes,
Joseph Ulie, Harvey Thomas, and Rudy Hodor of this office on April 28 through
June 6,1986, of activities at Clinton Nuclear Power Plant authorized by NRC
Construction Permit No. CPPR-137 and Materials License No. SNM-1886 and to the
discussion of our findings with Mr. J. Perry at the conclusion of the onsite
inspection.
The enclosed copy of our inspection report identifies areas examined during the
inspection including inspector in-office review. Within these areas, the
inspection consisted of a selective examination of procedures and representative
records, observations, and interviews with personnel.
During our inspection we determined that the majority of the features of the
fire protection program were complete at the time of the inspection visit,
although, implementation of your fire protection administrative controls has
yet to be fully implemented.
In addition, certain other fire protection
features which appeared once completed were observed incomplete due to
additional plant modifications such as removal of structural steel, sprayed on
fire resistive material, unsealed penetration openings in designated fire
barriers and fire doors having been found in the held open position.
During this inspection, certain of your activities appeared to be in
noncompliance with NRC requirements, as specified in the enclosed Appendix. A
written response is required. Further, as mentioned in this and a previous
inspection report, in addition to the specific deficiencies identified in the
Notice of Violation which are of concern, of equal importance to the NRC is
your control in ensuring proper installation and maintaining of the fire
protection features at the Clinton site.
In your response to the Notice of
Violation, please address the underlying cause that resulted in a second
progrannatic breakdown of not ensuring the proper fire protection features
were installed as required by your Materials License.
In accordance with 10 CFR 2.790 of the Conmission's regulations, a copy of
