We have reviewed your application dated February 11, 1986 and find we will need additional information as follows:

1.

The manufacturer's name and model number of the sealed source, and source holder or device on which the source will be used.

2.

Your statement in item six would suggest the gauge is intended for a permanent installation on a product line but item 3 states the gauge will be used at various locations.

Please state if the gauge is intended for a permanent installation.

If you are planning to remove the gauges for use at various locations we will need the following information:

a.

The addresses of the other locations of use, b.

A simple line sketch of the production line showing where the gauge to be located.

c.

The name of the person (s) who will be installing, removing and relocating the gauges.

d.

A description of the training this person has had with the use of the gauges and particularly the training that pertains to installation and relocation of the gauges.

e.

A copy of the procedures you will follow when installing, removing and relocating the gauges.

f.

A copy of the transportation procedures you will use if you are transporting the gauge to the various locations of use.

3.

Provide a description of the radiation safety program you will follow.

Address the following areas:

a.

Lock-out procedures to prevent persons from becoming exposed to radiation when the source shutter is open.

b.

Your leak test procedures, e.g., name and model number of leak test kit you will use, who will collect the leak test and how will you have it analyzed.

c.

Your emergency procedures in the event the gauge should become damaged and the source exposed. These procedures should include isolating the area, contacting your radiation safety officer and notification of the proper authorities, how will the damaged source be removed and by whom.

4.

Provide a description of the duties cf your Radiation Safety Officer.

These should include:

a.

To ensure that the use of radioactive material is by or under the direct supervision of individuals specifically listed on your license, b.

To ensure that radioactive materials are properly secured against unauthorized removal at all times when not in use.

c.

To ensure that the terms and conditions of your license are met, and that all required records are maintained.

d.

To inmediately halt any activity judged to be a threat to health, safety, the environment or a violation of the conditions of your license or the regulations.

We will continue our review of your application upon receipt of this information.

Please reply in duplicate, within 30 days, and refer to Control Number 80693.
