REGISTRY OF RADIOACTIVE SEALED SOURCES AND DEVICES SAFETY EVALUATION OF DEVICE NO..l NR-8094-D-805-S DATE: September 29, 1997 PAGE 2 OP 4 DEVICE TYPE:

Gauge Source Holder DESCRI!.10N:

The device is cylindrical in shape, 7-5/16 inches (185.7 mm) in height, and 4-1/2 inches (114.3 mm) in diameter with a 4-3/4 inch (120.7 mm) square base plate.

The steel base plate, 1/4 inch

( 6. 4 mm) thick, includes 4 mounting holes 5/16 inch (7.9 mm) in diameter.

The steel cylinder has a 1/8 inch (3.2 mm) wall and a top plate 3/16 inch (4.8 mm) thick.

These components are joined by welds.

The cylinder is filled with lead shielding, which includes two cavities.

A rectangular beam port, 1-1/4 inches (31.8 mm) by 1-5/16 inches (33. 3 mm) at. the outer surface of the device, tapers to its apex at the approximate position of the source in the "on" position.

The other cavity consists of a vertical hole lined with H_ inch (12.7 mm) inside diameter steel tubing.

This hole is 5-1/4 inches (133.4 mm) deep and is located on the vertical axis of the cylinder..The source is mounted at the bottom end of a source rod which is raised a distance of 2-7/16 (61.9 mm) inched to expose the source at the apex of the beam port.

The bottom portion of the source rod, 3/8 (9. 5 mm) inch in diameter and 2-1/4 (57.2 mm) inches in length, consists of a Mallory-1000 metal.

This is threaded to the upper portion of~the source rod consisting of a 1/4 inch (6.4 mm) square, steel piece topped by a knob.

A stop secured to the top plate by two screws has a 17/64 inch (6.7 mm) square hole allowing passage of the 1/4 inch square portion of the rod only.

This prevents removal of the source and positions it at the apex of the beam port.

A pin is used to " lock" the source in either the "on" or "off" position.

The pin is inserted through a. hole in the stop and a hole in the source rod.

A cable passing through the locking pin and looped at each end may be padlocked in place to prevent

- movement of the source.

REGISTRY OF RADIOACTIVF SEALED SOURCES AND DEVICES SAFETY EVALUATION OF DEVICE NO.: NR-8094-D-805-S DATEt September 29, 1997 PAGE 3 OF 4 DEVICE TYPE:

Gauge Source Holder LABELING:

A metal label 3-3/8 inches (85,7 mm) by 2-3/16 inches (55.6 mm),

attached to the outside of the device 180" from the radiation beam bears the following:

CAUTION:

RADIOACTIVE MATERIAL Cesium-137 mc Date of Measurement Model No.

02-0123 BRUN CORPORATION, COLUMBUS, OHIO The label is basically yellow in color with black lettering and a magenta radiation symbol.

A label attached to the device, opposite the beam port, indicates the source position as " SOURCE OPEN" and " SOURCE CLOSE" by arrows.

EXTERNAL RADIATION LEVELSt The radiation levels at one foot (304. 8 mm) from the surface of the device containing 50 millicuries (1.85 GBq) of cesium-137 are generally less than 1.5 mR/hr (15 ySv/hr) with a maximum of about 2-1/2 mR/hr (25 pSv/hr) at a fcot from the top of the surface of the device.

This includes radiation levels for both the "on" and "off" positions of the source.

LIMITATIONS AND/OR OTHER CONSIDERATIONS OF USEt Handling, storage, use, transfer, and disposal: To be determined by the licensing authority.

The device shall be leak tested at intervals not to exceed 6 months using techniques capable of detecting 0.005 microcurie (185 Bq) of removable contamination.

REVIEWER NOTE:

The Brun Corporation had stated that they would install, survef, and leak test each device.

They had also indicated a willingness to provide maintenance and repair services including relocation.

SAFETY ANALYSIS

SUMMARY

This device was originally approved for licensing purposes on May, 27, 1966, by the Atomic Energy Commission (AEC).

The Model 02-0123 Gauge Source Holder is no longer manufactured and distributed by Brun Sensors Systems.

Brun Sensor Systems materials license terminated January 23, 1973, and the company was acquired by Industrial Nucleonics that same year.

There is insufficient information on file for this device to perform a safety evaluation.

Therefore, this certificate is being taken to inactive status without further review based on the original review and approval issued by the AEC.
