WE have reviewed your license file pursuant to your request for termination dated January 16, 1987.

Upon review of your file, we found an inspection report covering an inspection completed on April 21, 1981. The report noted that a total of eight nuclear pacemakers had been implanted in patients during the years 1974 through 1976.

In order to complete your termination request, we will need documentation on the final disposition of all eight pacemakers. As long as even one pacemaker remains in a patient, your license must remain active.

Your institution is obligated to continue patient follow-up until the nuclear pacemaker is replaced with a non-nuclear unit or until the patient's death and subsequent recovery and disposal of the nuclear unit.
