SUBJECT: NRC ENGINEERING AND TECHNICAL SUPPORT INSPECTION REPORT

50-295/97018(DRS); 50-304/97018(DRS) AND NOTICE OF VIOLATION

Dear Mr. Brons:

On July 30,1997, the NRC completed an inspection at your Zion 1 and 2 reactor facilities. The

enclosed report presents the results of that inspection.

The inspection involved examination of engineering related activities; mostly restart issues.

Areas examined during the inspection are identified in the report. Within these areas, the

inspection consisted of selective examin9tions of procedures and representative records,

interviews with engineering staff, and observation of engineering related activities in progress.

Based on the results of this inspection, the inspectors determined that recent engineering

efforts and activities to identify and address long standing design deficiencies were more

comprehensive than past efforts. However, two violations of NRC requirements occurred.

These are cited in the enclosed Notice of Violation (Netice) and the circumstances surrounding

them are described in detall in the subject inspection report. Please note that you are required

to respond to this letter and should follow the instructions specified in the enclosed Notice when

preparing your response. The NRC will use your response, in part, to determine whether

further enforcement action is necessary to ensure compliance with regulatory requirements.

We are concerned about the violations of NRC requirements that were identified during this

inspection. The first violation involves the hilure to establish appropriate measures to control

parts or components which did not conform to design requirements in order to prevent their

inadvertent use or installation. For example, high efficiency particulate air (HEPA) filters

containing aluminum in excess of the design basis limits were installed in Units 1 and 2

containments in addition. nonseismically qualified solenoids were installed on the 1C and 2C

containment spray pumps. The second violation pertains to the failure to implement timely and

- effective corrective actions to address engineering deficiencies and related compensatory

measures. Examples included a 1992 operability evaluation which allowed operation of the

component cooling system down to 40*F (the system operating procedure only allowed

operation above 70*F) and contained compensatory actions which were not implemented by

engineering. Also, several instances of failure to incorporate design requirements into the

appropriate operating procedures in a timely manner were noted.
