SPECIAL REPORT DIESEL GENERATOR 1A VALID-FAILURE

On January 19, 1986 a valid failure occurred'on Diesel Generator

. i (D/G) 1 A.'

This valid failure was the fifth failure within the last 75 valid tests for the Catawba Unit 1 Diesel Generators.

Please

note that as.of January 19, 1986, the 1A'and 1B Diesel Generators had only completed 75 valid tests (this includes the five valid failures).

The failure occurred while the lA D/G was running in the fully

loaded condition.

During the test, the fuel supply make-up x'

solenoid valve failed to open and provide fuel for the 1A D/G day tank.

Thirty-three minutes into the test, the lA D/G tripped due s

to a lack of fuel.

To correct the malfunction, the lA D/G day tank manual bypass supply valve was opened to refill the day tank.

In addition, an auxiliary. fuel oil pump was used to refill the 1A D/G fuel supply piping.

The lA D/G was restarted and verified ope'rable-by performing the 1A D/G operability periodic test.

The 1A D/G was.

. declared inoperable on January 19, 1986 at 2245 and was t,

w subsequently declared operable on January 20, 1986 at 1600.

There fore, the 1A D/G was out of service for 17 hours1.967593e-4 days <br />0.00472 hours <br />2.810847e-5 weeks <br />6.4685e-6 months <br /> and 15

minutes.. At this time,-the surveillance testing interval is 3 days e,

which is in accordance with Section C.2.d.4 of Regulatory Guide-
