SEQUOYAH REQUALIFICATION EXAMINATION REPORT 327/0L-86-03 The NRC administered examinations on December 15-18, 1986, to employees of your company who hold licenses to operate the Sequoyah Nuclear Plant. At the conclu-sion of the written examination, a copy of the exam was left with your staff. At the conclusion of the site visit, preliminary findings on the operating examinations were discussed with those members of your staff identified in the Examination Report, Enclosure 1. Copies of the written examination questions and answer key are included in this report as Enclosure 2. Sequoyah facility comments regarding the written examination are included in this report as Enclosure 3. An evaluation of Sequoyah's requalification program is included in this report as Enclosure 4. This evaluation of the Sequoyah requalification program found the program to be marginal based on the performance of the operators on the written examination. NRC requests that TVA examine the written examination results to determine the cause of the marginal performance. TVA should also determine what corrective actions are needed to enhance knowledge of the failing operators and to correct any identified generic training deficiencies. Following TVA's evaluation, we request that you inform us of your findings and planned schedules for future actions. With regard to the individuals who did not pass the examination, it is our understanding that TVA has removed these individuals from licensed duties and placed them in an accelerated training program, as specified by your NRC approved requalification program. NRC intends to review TVA's written examination for suitability prior to its administration to the individuals to confirm the success of the accelerated training program.
