NOTICE OF VIOLATION Iowa Electric Company Docket No. 50-331 As a result of the inspection conducted on December 2-5, 9-12, 15-18, 1986, and January 12-16, 1987, and in accordance with " General Policy and Procedures for NRC Enforcement Action,* 10 CFR Part 2, Appendix C (1985), the following violations were identified.
1.
10 CFR 50, Appendix B, Criterion V, as implemented by Duane Arnold UFSAR, Section 17.2.5, requires that activities affecting quality be prescribed by documented instructions and procedures and accomplished in accordance with those instructior.s and procedures.
Contrary to the above. the licensee failed to perform use history evaluations for measu ing and test equipment found out of calibration as required by Proced.re No. MD-017.
This is a Severity Leiel IV violation (Supplement I).

10 CFR 50, Appendix E, Criterion VI, as implemented by the Duane Arnold UFSAR, Section 17.2.6.1, requires that documents, including changes, are reviewed for adequacy, are approved for release by authorized personnel, are distributed to an: used at the locations where the prescribed activities are perforned, and are controlled.
Contrary to the above. the licensee had not maintained a co:pleted and up-to-datesetofPipingandInstrumentationDiagrams(P&ID't)inthe control room for use Dy the reactor operations personnel.
This is a Severity Le.el V violation (Supplement I).

this office within thirty days of the date of this Notice a written statement (1) corrective action or explanation in reply, ir.clucing)for each violation:

further violations; and (3) the date when full compliance will be achieved.
Consideration may be giver to extending your response time for g:cd cause shown.
