APPENDIX A NOTICE OF VIOLATION Philadelphia Electric Company Docket No. 50-353 Limerick Unit 2 License No. CPPR-107 As a result of the inspection conducted on December 22, 1986 to February 2, 1987 and in acccordance with 10 CFR 2, Appendix C (Enforcement Policy 1987),

the following violations were identified.

A.

10 CFR 50, Appendix B Criterion VI and the Limerick Preliminary Safety Analysis Report Appendix D.4.13 require that measures be established to control the issuance of documents which prescribe activities affecting quality.

Contrary to the above, as of January 16, 1987, an unverified and unapproved document containing design and vendor data, which prescribed activities affecting quality, was created and issued to the Bechtel Electrical Construction Engineering Cable Group to provide input data for the performance of cable pull tension calculations.

This is a Severity Level V Violation (Supplement II).

B.

10 CFR 50, Appeadix B Criterion III and the Limerick Preliminary Safety Analysis Report Appendix D.6.2.1 require that drawings affecting quality are reviewed for technical adequacy.

Further, Bechtel Engineering Department Procer!ure EDP-4.58 requires that supplier documents and equipment are reviewed to assure compliance with the Bechtel design.

Contrary to the above, on January 16, 1987, shop welds on the Residual Heat Removal heat exchanger support gusset plates, were as shown on Bechtel approved vendor drawing 8031-C-41A-292-6, rather than as depicted on associated Bechtel design drawing C-196, Revision 13.

L This is a Severity Level V Violation (Supplement II).

Pursuant to the provision of 10 CFR 2.201, Philadelphia Electric Company is hereby required to submit to this office within 30 days of the date of the letter transmitting this Notice, a written statement of explanation in reply to the violations to include:

(1) the corrective steps which have been taken and the results achieved; (2) the corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending time.
