On November 22, 1985, we issued a letter to the Commonwealth Edison Company (CECO) under the provisions of Section 50.54(f),10 CFR Part 50, summarizing serious concerns we had with the operational and regulatory performance of the LaSalle County Nuclear Station. The Company was requested to develop and submit plans and programs to resolve the identified concerns. A corrective action plan was submitted by CECO in February 1986, and we have been meeting monthly to review the company's response. At the time of our acceptance of the response to the 50.54(f) letter, we indicated that we would review the company's progress on a monthly basis for six months, through August 1986, to determine what final action should be taken in response to the 50.54(f) letter. We have completed three of those monthly meetings to date.
The meetings have shown that progress has been made in terms of improving performance in certain areas as measured by a number of the parameters that were identified. Nevertheless, there remain areas which are still of concern to us, and I want to make sure I clearly identify these concerns to you as early as possible. The primary area of concern relates to the number of l
personnel errors which have occurred and continue to occur at LaSalle.
I can only conclude that the working level people at LaSalle are not committed to a f
goal of error free operation, nor are they convinced that they must perform their activities with careful attention to detail.
I want to bring this conclusion to your attention so you have a full appreciation of our concerns, and can take appropriate corrective actions to improve performance by the time we conclude our assessment.
We believe that the root cause of these problems is as follows:
1.
Plant management has not been effective in motivating personnel or in establishing management systems which will reduce personnel errors to a.
reasonable level on a consistent basis.
In saying this we recognize that LaSalle is a complex plant with multiple requirements imposed upon it.
2.
Corporate management has not required the necessary degree of accountability by the plant management and other personnel to assure quality operation. We sense that there is a level of acceptance of problems which is more geared toward average performance rather than toward excellent performance by both plant and corporate management.
