Submit a complete renewal application with proper fee, or termination request at least 30 days before the expiration date on your license. This letter is also requesting that Material 1.icense # 50-29015-01 be amended to reflect the following changes. 1. Item #1. Remove the name Arctic Slope inspection Senices completely from the title (see attached revised flow chart for your records). Your records show that Arctic Slope Inspection Services were exclusive to senicing the Alyeska Pipeline Service Company contract for radiogre.phy of the Trans-Alaska Pipeline. ASCG Incorporated, through the bidding process did not regain that contract and has subsequently lost the renewal of this contract and the existing contract expired on Febmary 5,1997. In the interim, ASCG Incorporated has transferred control of equipment (radiography equipment, vehicles, etc.) to ASCG Inspection Senices. There also is no likelihood that any contaminants remained after decommissioning of the facility as there were no . radioactive sources or sources of contamination keptin any stationmy or permanent facilitities there. All radioacive sources were kept in a locked (1 ton Darkroom Tnick that was stored in a fenced, locked storage yard located at 703 Bidwell Street., Fairbanks, Alaska. On the day of decommisioning, this truc was dispatched to ASCO,Inc. ,301 Arctic Slope Avenue, Anchorage, Ala was placed in a fenced, locked, storage yard immediately adjacent to t side of the building across an alleyway The radioactive source that had been in > storage in thi truck while it was in Fairbanks, Maska was then removed s Darkroom Trucks source storage box and placed into the source storage box i ' pemianent facility (ATCO Module) in the storage yard.
