INSPECTION PROCEDURE 40500 TEAM INSPECTION 50-293/97-80 AND
Dear Mr. Boulette:
From July 21 August 1,1997, Mr. W. Cook of this office conducted a Team Inspection at
the Pilgrim Nuclear Power Station, Plymouth, Massachusetts. The inspection was conducted
to evaluate the effectivuness of your processes for identifying, resolving, and preventing
issues that degrade the quality of plant operations and safety. The preliminary findings were
,
discussed with you and other members of your staff at the conclusion of the inspection.
Based upon the team's limited review, the overall implementation of your problem
identification, root cause, and corrective action processes appears to have improved since our
previous inspection in this area. The performance feedback provided by the safety oversight

groups and departmental self assessment functions appear to be generally good. However,
corrective actions for human performance related problems to the February 25,1997 reactor
scram, did not get entered into your Integrated Action Data Base tracking system for proper
tracking and implemantation due to a f ailure to follow administrative controls. In addition, two
other examples of failure to adhere to station procedures were identified. As identified via
your staff's common cause analyses and as noted in this inspection, procedural quality and
use continue to be the area of principle human performance concern. Although corrective
actions are near completion and there are some signs of improvement, line management and
the oversight function groups have not been completely effective in substantially improving
in the area of procedure quality and use.
With respect to the implementation of your Nucl oar Safety Concerns Program, the team's
observations of poor identification, organization, and tracking of issues, raises doubt on the
overall effectiveness of this process. As a confidential avenue and " safety net" to employees
or contract personnel who feel the normal problem reporting and corrective action processes
have not aatisfied their particular concern, this process's credibility appears to be severely
diminished. We request that you respond, to this letter and address the actions you have
taken or plan to take related to your Nuclear Safety Concerns Program.
