APPENDIX A Westinghouse Electric Corporation Docket No. 99900005/86-01 NOTICE OF NONCONFORMANCE During an inspection conducted on August 18-20, 1986, the implementation of the Westinghouse Quality Assurance (QA) Plan at the Specialty Metals Plant (SMP) in Blairsville, Pennsylvania was reviewed with respect to the fabrication of nuclear fuel tubing. Topical Report WCAP 8370/7800, " Westinghouse Water Reactor Divisions Quality Assurance Plan," Revision 10A/6A, dated August 1984, which was reviewed and approved by the NRC, describes the QA program of the Westinghouse Electric Corporation. Based on 'the results of this inspection and in accordance with the NRC Enforcement Policy of 10 CFR Part 2, Appendix C, the following items of nonconformance were identified.

Section 17.1.12 of WCAP 8370/7800, Revision 10A/6A, dated August 1984, requires, in part, that measuring and test equipment used for accepttr.ce and verification of quality be maintained under control systems and that documents detail the use of appropriately traceable measurement standards.

Section 6.12 of SMP Product Assurance Department Administrative Procedure PA-003, Revision C, requires, in part, that written procedures for control and calibration of measuring and test equipment used in activities affecting quality exist and that standards are purchased and verified using(measuring equipment which is traceable to the National Bureau of Standards NBS),

whenever possible.

Contrary to the above, the standard used in ultrasonic testing (UT) to validate tube ovality is not serialized and maintained under the equipment calibration control system nor are the dimensions of the standard traceable to NBS.

Section 17.1.5 of WCAP 8370/7800, Revision 10A/6A, dated August 1984, requires, in part, that activities affecting quality are accomplished in accordance with documented instructions, procedures or drawings.

Manufacturing Order (MO) 200, dated February 25, 1986 and M0 206, dated i

February 25, 1986, require, in part, that calibrations be completed and the data recorded on the appropriate forms by March 1986.

Contrary to the above, the data for vertical linearity, horizontal linearity and calibrated attenuation was not recorded on the calibration data reports, dated February 24, 1986 and Parch 27, 1986, for two UT flow detectors (serial numbers 33 and 40).

Section 17.1.6 of WCAP 8370/7800, Revision 10A/6A, dated August 1984, requires, in part, that obsolete or superseded documents are controlled to prevent inadvertent use.

Contrary to the above, procedures QS-213, QS-249, QS-261, and QS-262 contained references to QS-118 which became an obsolete procedure on July 7, 1985 when it was superseded by procedure PA-103.
