This refers to the inspection conducted by Ms. C. Abbate and Messrs. J. Craig, D. Lynn, and P. Prescott of this office on August 18-20, 1986, of your facility in Blairsville, Pennsylvania and to the discussions of our findings with Mr. Ron Cost and members of your staff at the conclusion of the inspection.
The purpose of this inspection was to observe fuel tube manufacturing activities, review ultrasonic testing (UT), procurement, and equipment calibration activities, and follow-up on Westinghouse corrective action cor,cerning items identified during previous NRC inspections. Areas examined during the inspection and our findings are discussed in the enclosed report. Within these areas, the inspection consisted of an examination of procedures and representative records, interviews with personnel, and observations by the inspectors.
During observatiun of your activities and review of your records, it.was found the implementation of your Quality Assurance (QA) program failed to meet certain requirements. Weaknesses were identified in the areas of confirmed traceability to the National Bureau of Standards (NBS) and serialization in the equipment calibration control system of the standard used for UT equipment to validate tube ovality, documentation of results of UT equipment calibration, and document control. Additionally, one utwsolved item was noted in the procedure used to verify tube ovality ultrasonically. While the procedure is technically accurate, it is unclear whether the requirement of Total Indicator Reading (TIR) is being met. The specific findings and references to the pertinent requirements are identified in the enclosures to this letter.
Please provide us within 30 days from the date of this letter a written stater.ent containing:
- 1) a description of steps that have been or will be taken to correct these items; ?) a descripcion of steps that have been or will be taken to prevent recurrence; and 3) the dates your corrective actions and preventive measures were or will be completed. Consideration may be given to extending your response time for good cause shown.
