APPENDIX A NOTICE OF VIOLATION Fhilddelphia Electric Company Docket 50-353 Limerick Generating Station (Unit 1)

License No. CPPR-107 As a result of the inspection conducted on March 30, 1986 to May 9, 1986, and in accordance with the revised NRC Enforcement Policy (10 CFR 2, Appendix C) published in the Federal Registe~r on March 8, 1984 (49 FR 8583), the following violations were identified:

10CFR50, Appendix B Criterion V requires that activities affecting quality be prescribed and accomplished in accordance with documented procedures.

Construction procedure CPrW-1, paragraph 5.18 states "All metal removing tools for use on stainless steel shall be color coded red."

Contrary to the above, on April 10, 1986, metal removal tools, including grinding wheels and files, that were not color coded red, were in use on safety related stainless steel piping.

Furthermore, two of the three l

metal removal tool issue stations had neither color coded tools for use on

stainless steel nor approved materials for use in color coding tools for use on stainless steel.

This is a Severity Level IV violation.

h The: inspector reviewed the corrective actions taken in response to the above violation. The use of properly color coded metal removal tools was observed, training of crafts and tool room attendants was done and the attributes of CP-W-1 are scheduled for audit by QA.

This violation is closed, such that a written response-is not required.

10CFR50, Appendix B Criterion V requires that activities affecting quality be prescribed and acconplished in accc.-dance with documented procedures.

The radiography proceoure RT-XG-2 requires. in paragraph 5.1.5.2, that an indication datormined acceptable, through other means (e.g., visual examin-ation) be recorded on the radiographic review form.

Paragraph 8.2 requires interpretation of each radiograph and disposition of the material examined.

Contrary to the above as of April 7,1936 the following conditions existed:

One area of a radiograph, 52-56, for weld HBB-218-2, FW52 contained an indication, which was determined to be acceptable, where visual examina-tion of the weld had not been performed.

One radiographic review form, applicable to EBB-203-1, FW5, did not indicate the disposition of the radiograph.

This is'a Severity Level IV violation.

For the second violation, pursuant to the provisions of 10 CFR 2.201, Phila-delphia Electric Company is hereby required to submit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is snown, consideration will be given to exten-ding this response time.
