Rainier, Oregon 97048- , _ 1 SUBJECT: NRC INSPECTION REPORT 50 344/97-03; 7217/97-03 AND NOTICE OF i VIOLATION l Dear Mrl Quennoz: An NRC inspection was conducted July 21-24,1997, at your Trojan Nuclear Plant. The . enclosed report presents the scope and results of that inspection. In addition, a telephone ' followup exit briefing was conducted on September 8,1997 to present the final NRC i resolution for the notice of violation issued in this report.
F ! - Major activities reviewed as part of this inspection included decommissioning and dismantlement work underway, preparations for the steam reformer project, pouring of the - concrete for a portion of the Independent Spent Fuel Storage Installation (ISFSI) pad, and a l: review of the 1996 annual reports for environmental monitoring and effluent releases. ! Based on the results of this inspection, the NRC has determined that one violation of NRC requirements occurred. This violation is cited in the enclosed Notice of Violation (Notice) ' and the circumstances surrounding the violation are described in detail in the enclosed
report. Although your efforts to identify'and investigate the survey activities which ' resulted in the violation were noteworthy, this violation contributed to the release of ' contaminated material from Trojan's radiologically controlled area. This is of concern to the NRC since the final survey that is conducted prior to the material being removed from the Trojan _ site would likely not have detected the low levels of contamination involved with ' this incident. .This is considered a failure of a key element of the overall health physics
program at Trojan designed to prevent the release of radioactive material from the site.

Because the problem may have existed for a period of time prior to discovery, in addition to the items specified in the Notice of Violation, you are requested to provide an analysis of . -the potential for releases of contaminated material from the radiologically controlled area-

during the period that the CM 11 low count rate alarm was incorrectly set. Please provide f ' a clear and reasonable basis for your estimate.
- Please note that you are required to respond to this letter and should follow the instructions
specified in the enclosed Notice when preparing your response. The NRC will use your
