9 NOTICE OF VIOLATION Illinois Power Company License No. NPF-55 As a result of the inspection conducted on December 16, 1986 through January 26, 1987, and in accordance with the " General Policy and Procedures for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985), the following violation was identified:

10 CFR 50, Appendix B, Criterion V as implemented by the IP Operational Quality Assurance Manual, Chapter 5 states in part: Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings... .

Procedure CPS No. 3302.01, Reactor Recirculation (RR), required the following:

Paragraph 1.0 defines the purpose of the procedure was to provide direction for placing the Reactor Recirculation (RR) system into various normal operational modes and to provide corrective action in the event of abnonnal system operation.

Paragraphs 8.1.1.1.9 and 8.2.3.1 require verification and documentation that reactor coolant temperatures and RR system flow are within technical specification 3.4.1.4. limits prior to RR pump startup and prior to recovery of an idle RR loop, respectively.

Contrary to the above:

1. On January 21, 1987, control room operators attempted to start RR Pump B on three seperate occasions without prior verification and documentation that the required flow and temperatures were within technical specifi-cation 3.4.1.4 limitation. Since entering mode 4 operation on January 5, 1987, six of eight additional RR Pump starts were performed in violation of the above requirements.
2. On January 21, 1987, the control room supervisor directed the bypass of a differential temperature interlock and started RR Pump B from an idle loop condition. Neither CPS No. 3302.01 nor any other document provided instructions that directed the bypass of the interlock.

These failures to follow procedural requirements and the failure to provide documented instructions are a Severity Level IV violation (Supplement I)

Notice of Violation 2 Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each violation: (1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further violations; and (3) the date when full compliance will be achieved.

Consideration may be given to extending your response time for good cause shown.
