SUBJECT:
NRC INSPECTION REPORT 50 271/97 05 AND NOTICE OF VIOLATION
Dear Mr. Reid:
On July 19,1997, the NRC completed an inspection at your Vermont Yankee reactor
I
facility. The enclosed report presents the results of that inspection.
During the seven week period covered by this inspection period, your conduct of activities
at the Vermont Yankee facility was generally characterized by safety conscious operations,
sound engineering and maintenance practices, and careful radiological work controls.
Based upon the inspectors observations this period, two violations of regulatory
requirements were cited. These violations are the consequence of poor personnel
performance in the conduct of routine plant activities and in the maintenance of the plant's
material condition, in addition, the manipulation of a wrong hydraulic control unit drain
valve by the plant operating staff was considered for enforcement; however, we
determined that the event occurred prior to your implementing corrective actions for a
similar violation described in NRC inspection report 50 271/97-04. We ask that in your
response to this letter you provide any additional actions you plan as a result nf this event.
The identification of material condition deficiencies by the NRC inspectors is of concern to
us, in your response to the attached Notice of Violation, please provide us with your view
of these performance observations, their apparent cause, and discuss any actions you plan.
Your continued attention to personnel performance issues at the station is warranted.
The violations are cited in the enclosed Notice of Violation, and the circumstances
surrounding th9 violations are described in detailin the enclosed report. Please note that
you are required to respond to this letter and should follow the instructions specified in the
enclosed Notice when preparing your response in your response, please address whether
the performance problems identified are indicative of a negative trend in overall
performance and any plans you have to address any trend identified. The NRC will use
your response, in part, to determine whether further enforcement action is necessary to
ensure compliance with regulatory requirements.
