This is in reference to your letter dated July 9, 1986 to terminate your byproduct material license. In order to complete our review, we need the following additional information:
During the review of this file,'it was noted that four pacemaker patients were being followed by the V. A. Los Angeles at the time of the November 8, 1983 inspection. Also, your July 9, 1986 letter indicates that four implants were conducted under this license.
In telephone conversations with Mr. Wetterau of your staff, it.was established that three explants were conducted as follows:

Medical College of Georgia June 21, 1984 2.
V.A.M.C., Loma Linda July 29, 1984 3.
V.A.M.C., Los Angeles May 27, 1986 Mr. Wetterau also stated that the fourth patient being followed was implanted l
by the Los Robles Clinic in Thousand Oaks, California. Apparently, the l
pacemaker was never under license SNM-1440. The last contact with the patient, who cannot be located, was in September of 1984 As we have no information in the file,regarding the fourth pacemaker patient, you should submit information to document the licensure and. disposition of this source. A letter'from Los Robles Clinic stating that the pacemaker was implanted under their state license and that every effort has been made to t
contact the patient (including efforts to contact the patient's sponsors) should be included in the response.
