Gentlemen:

Subject: Inspection Report 50-311/86-26

This refers to a special team inspection led by Dr. L. Bettenhausen of this

office, on August 26, 1986 to August 30, 1986, at the Salem Nuclear Generating

Station, Hancocks Bridge, New Jersey. The inspection was conducted to deter-

mine the cause of the false loss of offsite power signal which occurred on

August 26, 1986 and to review actions during and following the event. The

findings of this inspection were discussed with Mr. J. M. Zupko, Jr. of your

staff.

Within the scope of this inspection several unresolved items were identified

as discussed in the enclosed inspection report.~ You are requested to review

the unresolved items and respond in writing within 30 days of the date of this

letter identifying your proposed actions, where appropriate. In particular,

we are concerned about the adequacy of the.10 CFR 50.59 review process that

apparently allowed the addition of electrical loads without determining their

effects on the transient response of the electrical system. In your reply to

the identified items please include the expected date of completion of your

actions.

Further, the preliminary results of your analysis of the August 26, 1986 event

were discussed with the Region I staff at a meeting on August 30, 1986.

Commitments made at this meeting regarding actions taken to justify continued

operation of Salem Unit 1 and the restart of Salem Unit 2 were described in

your letter to NRC Region I dated August 31, 1986. Your letter also committed

to provide monthly status reports regarding the long term actions planned.

The responses requested by this letter are not subject to the clearance

procedures of the Office of Management and Budget required by the Paperwork

Reduction Act of 1980, PL 96-511.
