NRC INTEGRATED INSPECTION REPORT 50 395/97 07
NOTICE OF VIOLATION-
Dear Mr. Taylor:
On July 26, 1997, the NRC completed 'an inspection at your V. C. Summer _ reactor-
facility.- The enclosed report presents the results of that inspection.
_
--During the 6 weeks covered by this inspection period, your conduct of
activities.at the V. .C.: Summer _- facility was generally observed to be -safe and
conscientious in the areas of operaticns, maintenance, engineering,_ and plant
support. We are concerned.: however, about the violations of NRC requirements
that were identified. .In the first instance, you failed to recognize the
significance of o
' while operating. pening the hydrogen monitor containment-isolation valves
In the second instance, you missed several opportunities.to
identify and correct procedural deficiencies in the snubber replacement
project.
These violations are cited in the enclosed Notice of-Violation (Notice),- and
the circumstances surrounding the violations are ' described in detail in the
enclosed report.
Please-note that you are required to respond to this letter
and should follow the instructions-specified in the enclosed Notice when
preparing your response. The NRC willLuse your response,-in part, to
determine whether further enforcement action-is:necessary to ensure compliance
with. regulatory requirements.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of

this letter and-its enclosures will be placed in the NRC Public Document Room.
