This inspection was conducted by NRC inspectors and contractors during the periods of

Februtry 18 through Juno 18,1997. The inspection included an assessment of the

progress made by the licensee in the review and corrective action for the cable separation
I
and Generic Letter 96-01 logic testing issues discussed in Confirmatory Action Letter (CAL)

96-01, dated December 18,1996, and supplement, dated January 30,1997. The CAL
documented a number of cable separation and testing problems you had identified prior to
the initial CAL issue. The CAL required you to expand your review to determine the extent

of the cable separation problem to ensure that all safety significant cable separation

problems were identified and appropnately dispositiened or resolved pursuant to your
licensing and design basis.

The CAL also required you to perform root cause evaluations that will address all hardware

your corrective actions. This inspection report is an update on those activities which had

also been discussed in inspection report 50-309/96-14 and does not close the NRC review
on either of these items.
The inspectors concluded that the methodology used to develop circuit specific test

drawings and compare those circuits with the test procedures was good and demonstrated

a good understanding of the problem. The response to the Generic Letter 96-01, Logic

Circuit Testing, will remain open pending NRC review of the results of your testing using

the permanent plant test procedures as discussed in Section E1.1 of this report.

The inspectors concluded that your response to the cable separation problem was

fragmented and lacked sufficient foresight in both scope, methodology and acceptance
}]
Giteria. The issue of cable separation remains open pending your resolution of the
separation discrepancies identified in your reviews, in addition, an additional inspection

followup item was opened. This open item involves your proposed submittal of a revised

cable separation criteria, and the NRC review, of your revised approach for cable

separation and review of any corrective actions that may be involved. This is discussed
further in Section E3.1 of this report.

Inspection reports 50-309/9611 [EA 96 320] and 9616 [EA 97-0341 discussed a number
..
. of apparent violations concerning equipment operability and testing inadequacies. An
enforcement conference was held on March 11,1997, with the Maine Yankee staff to
. discuss those apparent violations. - Enforcement action is pending 'on those items.
The initial NRC effort involved the use of two NRC contractors. NRC discovered a
potential conflict of interest with one of those contractors. Therefore, this report is only
based on the work documented by the second contractor and additicaal followup work
performed by a Region I specialist. The regional inspector compared the work performed
. by the two contractors for independence and independently verified selected items covered
by this report.
