SUBJECT:
NRC INSPECTION REPORT 50-361/97-11; 50-362/9711 AND
NOT'CE OF VIOLATION
Dear Mr. Ray:
An NRC inspection was conducted June 213 and August 4 7,1997, at your San Onofre
Nuclear Generating Station, Units 2 and 3, reactor facilities in-office. Following the
onsite exit meeting on June 6,1997, the results of this inspection were discussed with
Mr. R. W. Krieger of your staff vn June 13 and August 20,1997. The enclosed report
presents the scope and results of that inspection.
During this inspection, one violation of NRC requirements was identified. This
was identified by your staff and reported to the NRC in Licensee Event
Reports 50 361; 362/9609 00 and 01; 970100, 01, 02, and 03; and discussed in NRC
[
Inspection Reports 50 361; 362/96-18,97 02, and 97 09. The violation is of concern in
that it resulted from a lack of attention to detail, as well as inadequate management
attention and oversight, when implementing the new standard Technical Specifications and
resulted in Southern California Edison requesting Notices of Enforcement Discrotion on
three occasions. A number of examples were cited in which the current surveillance
requirement implementing precedures were inadequate to meet the new Technical
Specification requirements as adopted by your facility on August 5,1996.
The NRC recognizes the limited actual cafety significance of the individual examples
identified by your staff and that all equipment was demonstrated to be operable when the
equipment was rotestad in accordance with the correct surveillance requirements. Thus,
the violation has been classified at Severity LevelIV in accordance with the, " General
Statement of Policy and Procedure for NRC Enforcement Actions," NUREG 1600. We also
recognize your comprehensive effort involved in identifying and correcting the inadequate
surveillance requirements. However, given the large number of examples, the preventable
nature of these examples had you properly implemented the Technical Specification
improvement program, and the significance of the Technical Specification surveillance
program, we have determined that a Notice of Violation is warranted.
One violation, with several examples, is cited in the enclosed Notice of Violation (Notice).
The circumstances surrounding the violation are described in detailin the enclosed report

The NRC has concluded that information regarding the reason for the violation and
the corrective actions taken and planned to correct the violation and prevent recurrence
is already adequately addressed on the docket in Licensee Event Reports 50 361;
362/9609 00 and 01; 970100, 01, 02, and 03; and in NRC Inspection Reports
50 361; 362/9618,97 02, and 97 09. Therefore you are not required .to respond to this-
.
letter unless the description therein does not accurately reflect your corrective actions or
your position, in that case, or if you choo:;e to provide additional information, you should
follow the instructions specified in the enclosed Notice.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter,
its enclosure, and your response will be placs d in the NRC Public Document Room (PDR).
To the extent possible, your responce should not include any personal privacy, proprietary,
or safeguards information so that it can be placed in the PDR without redaction.
