- Nature of Defect and Safety Hazard:

Improper grounding of a transistor can cause an over-voltage with consequential failure of the transistor. This can cause a failure of proper load sequencing on startup of the plant diesel generator (s).

- Date Information Obtained:

April 10, 1986

- Number of Units Containing Defect:

Six units shipped to APS.

They are a one-of-a-kind design.

Corrective Actions Supply APS with a kit to ensure grounding of the transistors o

affected. Revise drawings accordingly.
