REQUEST FOR ADDITIONAL INFORMATION (TAC NO. M88345)

Dear Mr. Claassen:

We are continuing our review of your request for renewal of Facility Operating License No. R-57 for the Omaha Veterans Administration Medical Center TRIGA Research Reactor which you submitted on May 10,1993, as supplemented. During our review of your renewal request, questions have arisen for which we require additional information and clarification. Please provide responses to the enclosed Request for Additional Information within 90 days of the date of this letter. Following receipt of the additionalinformation, we will continue our evaluation of your request.

In accordance with 10 CFR 50.30(b), your response must be executed in a signed original under oath or affirmation. If you have any questions regarding this review, please contact me at (301) 415-1127.

Plant Description. What are the use factors for this facility? Is the facility used at night or on the weekend?

Plant Description. Figure 1.2 is missing. Please provide this figure. Also, please provide a figure that shows how the reactor is connected to the Main Medical Center Building.

3.

Organization and Responsibilities. How can the reactor supervisor serve simultaneously as emergency director, emergency coordinator, and radiation safety officer?

Emergency Action Levels. Do you expect to declare an NOUE for any fire occurring

anywhere? At what radiation level would you declare an NOUE based upon fuel damage? At what point are notifications made to the NRC and the local authorities?

Maintenance. Where is the emergency equipment maintained? How often do you check and calibrate the radiation detection instruments?

6.

Maintenance. At what frequency is the emergency plan reviewed and updated? The date of this emergency plan is 1984. 'The plan should be reviewed every two years.

Where are the updated letters of agreement between the facility and the local support agencies?

Operator Requalification Plan Questions 7.

In Section 2.4 " Evaluation", it is noted that an annual written examination is required. For your information the regulations in 10 CFR 55.59 only require an annual operating examination, and written examinations need to be given at least every two years. The regulations, however, do not preclude giving written examinations every year.

8, it is noted that the requalification plan does not state that an annual operating examination will be given, which will sample the applicable topics of 55.45(a)(2-13). Please revise the requalification plan to require that an operating test be given annually, and include applicable topics of 55.45(a)(2-13) that will be evaluated for operators and senior operators.

Section 2.6.1 should be revised to allow an exemption for the Reactor Supervisor

-(RS) from the written and operating tests only when the RS gives the examination.

The RS should not give two consecutive written or operating examinations and should be required to take the examinations on alternate iterations of the exams, 10.

Please retype the plan as a complete document, rather than add-on sections such as in sections 2 and 2.1. Also, the typos in section 2.3 and 2.3.3.2 should be corrected.
